Overview
Home Health Services provide comprehensive, medically necessary care to individuals in the comfort and safety of their own homes. These services are designed for patients who are recovering from an illness, surgery, injury, or managing chronic health conditions that require consistent medical attention but do not warrant hospitalization or admission to a long-term care facility. The goal is to promote healing, improve quality of life, and support the highest level of independence possible.

Target Population
Home Health Services are ideal for individuals who:

• Are recently discharged from a hospital or rehabilitation facility

• Are recovering from surgery or an acute illness

• Have chronic medical conditions such as heart disease, diabetes, or COPD

• Require skilled nursing, therapy, or monitoring at home

• Have mobility limitations or other barriers to accessing outpatient care

Core Services Offered
Our Home Health Program offers a multidisciplinary approach to care, with services tailored to meet the unique needs of each patient. Key offerings include:

• Skilled Nursing Services
  Assessment and management of medical conditions, wound care, catheter care, injections, IV therapy, and medication management

• Medical Social Work
  Support with accessing community resources, mental health services, counseling, and discharge planning

• Home Health Aide Services
  Personal care assistance including bathing, grooming, and light housekeeping to support the patient’s dignity and comfort

Care Coordination
Each patient’s care is managed by a licensed clinician who develops a personalized care plan in collaboration with the patient's primary care provider, specialists, and caregivers. Routine progress evaluations and updates ensure alignment with the patient’s health goals.
